Personalised Care Plans and Transitional Plans  
Each individual’s journey begins with a thorough assessment of their needs, abilities, and personal goals. Based on this, we work with the young adults and the people close to them to create a personalised care plan that focuses on their care and support needs. Alongside this, we create a transitional plan which focuses on goal setting and skill building in areas such as managing daily tasks, budgeting, cooking, and personal care.

 

Skilled, Compassionate Care 
Our dedicated team provides ongoing support, helping the young adults build confidence while encouraging them to take greater responsibility in their day-to-day lives. This balance between support and independence allows individuals to grow at their own pace, while feeling secure in knowing that help is always available when needed. 

 

In addition to our comprehensive staff training which is all staff receive to deliver high quality care, each staff member is also an expert within a life skills area. Staff members receive additional training in their expert area allowing them to pass this knowledge and expertise to the young adults they support.

 

Community Integration
As part of the transitional process, we encourage the young adults to engage in the local community, including work placements, volunteering, or social activities. This integration helps build the social skills and networks needed for more independent living, while promoting a sense of belonging and purpose.

 

Stepping Towards Independence
As the young adults become more proficient in various life skills, we gradually adjust their level of support, offering them more freedom and responsibility over time. This progressive approach ensures a smooth transition from a fully supported environment to a more independent lifestyle. Bridge House will assist young adults in finding their next living arrangements and provide ongoing support throughout the entire transition process.
